If I remember what I've heard about the games, only one is really his sister, the other 11 are adopted by his parents and sent to find him.

Agreed, from what it seems, Karen is his only true sister (though im guessing based on looks).
Either that, or the rest of them are just like, half-sisters. Could be either, not really clear.
