I bought 18 x 8 +42 wheels for my altezza, im going to trim the inner metal arch/guards at the rear what width spacer do i need to bring the wheels right to the edge guys?

I'd say about 20-25mm will have it fairly flush :)

Im running 18x9.5 +42 offsets on the back and it sits flush at the height im running (no spacers). All depends on how much camber you have. id be leaning towards the 25mm...

Providing my calculation are right....

Yeah I have 18x9 +45 on the rear and would love to throw a 15/20 mm spacer on it still too

I read that spacers makes the wheel bearings work harder? Is it a case of the thicker the spacer the harder bearings work?
